Everyone must have a Peru State College issued identification card (ID) to enter the facility. Students and college employees are eligible for one free ID card. Replacement cards or cards for non-students/employees are available for a fee set annually by the Board of Trustees, for 2020/21 that fee is $25.
GENERAL INFORMATION
The Peru State Fitness Center is an ADA accessible facility. All users must have a valid ID card to enter the facility. An ID card (or access via your faculty/staff/student ID card) is issued before completion of this Agreement. This Agreement must be completed annually and is valid August 1 through July 31 yearly. Any non-PSC student under the age of 18 must be accompanied by an adult.
POLICIES
General Use
*Participants exercise at their own risk.
*Users must be healthy enough to exercise. If you have any injuries or questions regarding your ability to exercise, consult your health care provider.
*Users must take the initiative to learn the proper use of equipment. It is strongly recommended that you are properly oriented on all exercise equipment prior to beginning an exercise program.
*Proper athletic gear must be worn at all times, which includes closed-toe shoes and shirts. Pants with belts or metal rivets and cut-off pants are not allowed.
*Water is allowed, but other food, beverages and gum are not permitted.
*Personal items, bags and other items are to be secured in locker room lockers only and not on the Fitness Center floor. The College is not responsible for lost or stolen items.
*Equipment cannot leave the facility.
*Bicycles, skateboards, roller skates and roller blades are prohibited.
*Pets are not permitted, except those providing assistance to individuals with special needs.
*No vulgar language or music. The sound system volume in the Weight Room will be controlled by the Fitness Center supervisor.
*Entrance must be made from the locker room or west entrance door using a valid ID card. Entry into the fitness center or weight room when closed will result in a $50.00 fine and loss of fitness center access.
*Propping doors open or allowing entry by anyone who has not signed a fitness center agreement is prohibited and may result in your loss of access to the fitness center.
*College policies regarding conduct, alcohol, tobacco, illegal substances and firearms will be enforced.
*Individuals are required to pick up after themselves, discard trash and remove personal items.
*Report all injuries, losses, damage and/or any equipment failures to the supervisor on duty, the Athletic Department at 402-872-2350 or Security at 402-872-2411.
Cardio Equipment
*During peak hours, or while others are waiting, limit your time on cardio equipment to 30 minutes.
*Properly wipe down equipment after each use.
Weight Training Equipment
*Replace all dumbbells on the appropriate racks in the assigned spaces.
*Due to the potential risk of injury, use a spotter when training with free weights.
*Do not drop free weights on the floor or allow weights to crash together.
*Allow other users to “work in” or take turns. Do not monopolize several pieces of equipment while others are waiting.
*Properly wipe down equipment after each use.
*Do not lift more weight than is comfortable and safe.
*Collars are required when using Olympic and curl bars.
USER AGREEMENT
I understand that, by checking the box below, my use of the Peru State Fitness Center facilities, equipment, and services (collectively, the “Facility”) includes inherent risks that no amount of care, caution, instruction or expertise can eliminate. I acknowledge that the use of the Facility may subject me to the risk of serious bodily injury, property damage or other danger. I freely assume such risk including serious bodily injury and/or death that may be sustained by me as a result of my use of the Facility, whether caused by the negligence of Peru State College, its agents and employees or otherwise. On behalf of myself, my personal
representatives, heirs, assigns and next of kin, I hereby release, waive, and forever discharge the Board of Trustees of the Nebraska State Colleges, the Nebraska State College System, Peru State College, and all
Trustees, administrators, agents, employees, and students (together, the “Released Parties”) from any and all liability, including claims or lawsuits related to any property damage, property loss, bodily injury (including but not limited to, loss of life), or any other loss related to use of the Fitness Center facility. I understand that I am responsible to pay, or otherwise cover through insurance, any medical or hospital expenses, doctor bills or other expenses which could be incurred as a result of my use of the Fitness Center facility.
I agree to exercise special care to avoid damage to the Facilities and the property of others. I agree that the Released Parties shall not be responsible or liable for the loss or theft of property from the Facility, any lockers or any storage areas.
I agree to abide by all of the policies issued with respect to the Facility and any instructions given to me regarding the use of the Facility. I agree that permission to use the Facility may be withdrawn at any time and for any reason or no reason.
My signature below confirms my consent to, and my compliance with this Agreement. A copy of this document will have the same effect as the original.
